L'endométriose pelvienne profonde est une pathologie féminine qui entraîne le plus souvent des douleurs pelviennes chroniques, inconstamment des troubles digestifs et urinaires. Ainsi elle retentit nettement sur la qualité de vie globale et sexuelle des malades. La prise en charge chirurgicale intervient après un bilan d'imagerie précis. Quatre vingt patientes ont été prises en charge par coelioscopie pour une endométriose pelvienne profonde. Avec un recul moyen de 27 mois après l'intervention, il existe une diminution significative de l'intensité des symptômes et une amélioration franche de la qualité de vie et de la sexualité. En revanche ces résultats suggèrent que ce type de chirurgie est pourvoyeuse de troubles urinaires de novo de type rétentionnel, probablement secondaire à des lésions nerveuse pelviennes. Cette chirurgie doit donc ètre pratiquée en respectant au mieux les plexus nerveux.
